What is OET Nursing?

English testing built for clinical nurses.

The Occupational English Test (OET) is a profession-specific English exam recognised by 60+ healthcare regulators worldwide, including the NMC. There are 12 OET professional versions; OET Nursing is the version designed for registered nurses, midwives and student nurses.

Unlike IELTS Academic, every part of OET Nursing uses real nursing content — patient case notes, ward handover dialogues, clinical guidelines, and nurse-patient role-plays. For overseas nurses, this clinical context typically makes the test feel more natural than IELTS.

OET Nursing also satisfies the UK Skilled Worker visa English language requirement (CEFR B2). A Grade B in OET Nursing means you do not need to take a separate visa-stage English test. See our CEFR B2 guide for details.

NMC: Grade B required

NMC sets the highest evidence bar: Grade B (350+) in Listening, Reading and Speaking, plus Grade C+ (300+) in Writing. Slightly above the OET requirement of most other regulators.

Profession-specific content

Writing tests producing a discharge or referral letter from real case notes. Speaking tests ward-based role-plays. Listening and Reading use clinical content but a common format with all OET professions.

Resits per sub-test allowed

OET allows you to retake only the sub-tests where you missed your target — within 12 months you can combine best scores. This makes a focused resit strategy realistic if you miss only one sub-test.

Listening (45 min)

Common for all professions. Three parts: consultations between practitioner and patient (Part A), short workplace dialogues (Part B), and presentations or interviews (Part C). 42 items total. Target: 350+ (Grade B).

Reading (60 min)

Common for all professions. Three parts: rapid scan of short health texts (Part A), workplace-based texts with MCQs (Part B), and longer evidence-based articles (Part C). 42 items total. Target: 350+ (Grade B).

Writing (45 min)

Profession-specific. Nurses write a referral, discharge or transfer letter based on case notes. Tested on language accuracy, content selection, clarity, organisation and reader-focused tone. Target: 300+ (Grade C+).

Speaking (20 min)

Profession-specific. Two role-plays: you play the nurse, the interlocutor plays a patient or carer. Scenarios reflect real ward situations — explaining medication, handling distress, breaking news, escalation. Target: 350+ (Grade B).

Score requirements

What you need to score for NMC registration.

The NMC sets one of the tougher OET Nursing thresholds — Grade B in three of the four sub-tests. Listening, Reading and Speaking must all score at 350 or above. Writing must score at 300 or above (Grade C+). You can combine sub-test results from two sittings taken within 12 months.

Once you have your NMC OET Nursing result, that same result also satisfies your UK Skilled Worker visa English language requirement (CEFR B2). A single OET sitting solves both the regulator and the visa step.

Weeks 1–2

Diagnostic & format

Sit one full mock OET to identify your current grade in each sub-test. Read the OET official handbook end-to-end. Learn the exact mark scheme — knowing exactly how examiners score is half the battle.

Weeks 3–4

Listening & Reading drill

Daily Part A, B, C practice (45 min each) for Listening and Reading. Use only OET-style content (medical and nursing podcasts, NMC and NHS news). General English is not enough — clinical context is the differentiator.

Weeks 5–6

Writing — case notes to letter

Practise turning OET case notes into a referral letter for 30 different scenarios. Drill paragraph structure (purpose, history, current findings, request). Focus on selecting relevant information — most candidates lose marks on content selection, not language.

Week 7

Speaking role-play practice

Record and review 20+ role-play scenarios with a study partner. Patient-centred language, empathy, signposting and checking understanding all carry marks. Avoid clinical jargon and never read from a script.

Week 8

Full mock exams + targeted revision

Two full mock OETs under timed conditions. Identify the 3 weakest scoring categories from the mock and target only those areas in your final week. Reduce study volume and rest 2 days before the test.

Use nursing-specific content from day 1

General English exam materials are not enough. OET is built around clinical scenarios — patient case notes, ward dialogues, NMC standards, and nursing handovers. Use NMC newsletters, NHS England nursing podcasts, and nursing journals as study material.

Master the OET letter structure before you write content

Most Writing failures come from poor structure, not weak grammar. Memorise the four-paragraph referral letter template (purpose, relevant history, current admission and findings, request) and drill it on 30+ case notes.

Speak naturally — assessors penalise robotic role-plays

Speaking is scored on real communication skills: empathy, signposting, checking understanding, building rapport. Reading from a memorised script almost guarantees a Grade C or below. Practise with a partner who reacts naturally.

Use the OET handbook mark scheme — not IELTS habits

OET Writing rewards content selection from case notes ("Did you include the right information?") in a way IELTS does not. OET Speaking rewards patient-centred language. Past IELTS strategies do not transfer cleanly — start fresh.

Book Computer-Based OET if writing fast is a weakness

OET is offered in both paper-based and computer-based formats. Many nurses with weaker handwriting speed prefer the computer-based version — it can also be re-taken within a shorter window. The mark scheme is identical.

Aim for Grade B in all four sub-tests at one sitting

OET allows you to combine the best score for each sub-test across two sittings within 12 months. But the cleanest path for NMC is a single sitting with B in Listening, Reading, Speaking and C+ in Writing. Resits cost ~£587 each.

How long is OET Nursing valid for?

OET test results are valid for 2 years from the test date for NMC registration purposes. You must submit your NMC application within this window. The 2-year clock does not reset on minor re-applications.
