PLAB from Pakistan in 2026: The Complete Route from MBBS to an NHS Job

More doctors sit PLAB from Pakistan than from almost anywhere else, and the route is well worn: MBBS, house job, English test, PLAB 1 in Islamabad, Karachi or Lahore, PLAB 2 in Manchester, GMC registration, an NHS job and the Health and Care Worker visa. What is less well understood is where Pakistani applicants specifically lose time — a house job that does not evidence the rotations the GMC wants, an English route that does not exist for Pakistani graduates, a degree verification that sits with a university for months, and a visitor visa refused on funds.

This guide walks the whole route as it stands in 2026, in the order the steps must happen, with the fees in pounds and approximate rupees, the timeline, and the Pakistan-specific traps at each stage. It links out to our detailed guides on each exam rather than repeating them.


The short answer

  • Eligibility: an MBBS from a PM&DC-recognised college listed in the World Directory of Medical Schools, plus a completed twelve-month house job with at least three months each in medicine and surgery.
  • English: IELTS Academic 7.5 overall with 7.0 in every band, or OET Medicine grade B in all four sub-tests, in a single sitting. Plan on sitting one — the "taught in English" route rarely works for Pakistani graduates.
  • Exams: PLAB 1 (£283) at British Council centres in Pakistan, then PLAB 2 (£1,036) in Manchester on a £135 Standard Visitor visa.
  • Cost and time: roughly £2,500–£3,500, or around PKR 0.9–1.3 million at approximately PKR 375 to the pound, and 12–18 months from booking PLAB 1 to a licence to practise.
  • Then the job: most newly registered Pakistani doctors start in trust-grade or clinical fellow posts and move into training from inside the NHS.

Step 1: check your degree and your house job

The GMC accepts a primary medical qualification that appears in the World Directory of Medical Schools and meets its criteria — in practice, every PM&DC-recognised MBBS from a public or private college in Pakistan. The degree itself is rarely the problem. The house job is.

For full registration the GMC needs evidence of an acceptable internship: at least twelve months of continuous, supervised clinical practice in a hospital recognised for training, completed immediately before or after graduation, including at least three months in medicine and three months in surgery. Pakistan's one-year house job satisfies this on paper, but the GMC checks the detail, and three problems recur:

  • Rotations that do not add up. A house job spread across four three-month specialties with no general medicine or general surgery block, or with a paediatrics or gynaecology rotation the GMC does not count as medicine or surgery, can fall short. Check your rotations against the GMC's internship checker before you spend anything on exams.
  • Gaps. Breaks between rotations, or a house job completed in two hospitals with a gap between them, break the "continuous" requirement and need explaining with evidence.
  • Weak paperwork. The GMC wants a signed letter from the hospital listing every rotation with start and end dates and confirming supervised practice with direct patient contact. A generic completion certificate is not enough. Get the detailed letter now, while the medical superintendent's office still knows you.

If your house job cannot be made to fit, the GMC can instead accept evidence of two years of continuous supervised practice after graduation in an appropriate hospital, with adequate medicine and surgery experience — but that is a slower route and needs planning.

Step 2: English — do not rely on your MBBS

Pakistani medical colleges teach and examine in English, and many graduates assume this exempts them from a language test. It almost never does. The GMC's alternative evidence route requires the medical school to confirm that at least 75% of clinical interaction with patients was conducted in English, which Pakistani hospitals generally cannot truthfully certify, and it is only available within two years of graduation. Plan on a test.

Results are valid for two years for GMC purposes, so sit the test no earlier than about eighteen months before you expect to apply for registration. Step 3: PLAB 1 in Pakistan

PLAB 1 is the written exam — 180 single-best-answer questions over three hours, written to the MLA content map — and it can be sat in Pakistan. The GMC runs overseas sittings at British Council venues in Islamabad, Karachi and Lahore, usually in February, May, August and November, for a fee of £283 (about PKR 106,000) paid to the GMC through GMC Online. Before you can book you need a GMC Online account, your English evidence on file and your primary qualification details entered.

Two Pakistan-specific realities. First, seats at the Pakistani venues are heavily oversubscribed; booking windows open weeks before each sitting and the popular centres fill within hours, so be logged in when the window opens and have a second city as a fallback. Second, a pass is valid for two years, within which you must pass PLAB 2 — so do not sit PLAB 1 until you are ready to fund and plan the Manchester trip within the following twelve months. Step 4: PLAB 2 in Manchester, and the visa that gets you there

PLAB 2 is a 16-station clinical exam held only at the GMC's assessment centre in Manchester, for a fee of £1,036 (about PKR 389,000). You book it through GMC Online once your PLAB 1 pass is recorded, and dates in demand should be secured three to six months ahead. To sit it you need a UK Standard Visitor visa: £135 for six months from April 2026, applied for online with biometrics at the visa application centres in Islamabad, Lahore or Karachi, and decided in around three weeks. For Pakistani applicants this is the most common point of failure on the entire route, because a visitor visa is decided on funds, ties and credibility, and a recently graduated doctor with a small balance and no job is exactly the profile caseworkers scrutinise. Step 5: registration evidence — start this before you fly

Registration is not a formality after PLAB 2. Three pieces of evidence take time, and Pakistani applicants who leave them until after the result routinely add three to six months to the route.

EPIC is the one to start earliest. Verification requests to Pakistani universities are handled by registrar and examination offices that are not built for it, and it is common for a request to sit for months without a reply. Start EPIC around the time you book PLAB 1, follow up with the university in person if you can, and keep the reference numbers. Once PLAB 2 is passed and the evidence is in, you apply for full registration with a licence to practise through GMC Online, pay the registration fee (£156 if your degree was awarded less than five years ago, £406 otherwise), and attend an in-person identity check in the UK before the licence is granted. After registration: the job, and the visa that follows

Since the Medical Training (Prioritisation) Act 2026, UK graduates are offered foundation and specialty training posts first, and most newly registered international doctors — Pakistani doctors very much included — start in non-training posts: trust-grade SHO, junior clinical fellow or stand-alone FY2 roles advertised on NHS Jobs. With a job offer and a Certificate of Sponsorship you apply for the Health and Care Worker visa from Pakistan — £324 for up to three years from April 2026, no Immigration Health Surcharge, and this time a TB test certificate is required because the stay exceeds six months. Dependants can accompany you on this route, unlike the Student and care worker routes that have been restricted.

The five Pakistan-specific mistakes

  1. Assuming the house job is fine. Check the rotations and get the detailed letter before booking anything.
  2. Waiting on the English exemption. Weeks lost to an application the GMC will not accept. Book IELTS or OET.
  3. Sitting PLAB 1 casually. It starts a two-year clock and uses one of four attempts; sit it when the PLAB 2 trip is fundable.
  4. Starting EPIC after PLAB 2. The single biggest avoidable delay for Pakistani graduates.
  5. Under-documenting the visitor visa. A weak bundle can cost you a Manchester date and, on a late cancellation, the full £1,036 fee.
